How we handle your information.
We collect the minimum we need to do the work, we tell you what we have, and we never sell it. The plain-language version is on this page. The full legal language is below.
§ 01Who we are
Dyad Health, Inc. ("Dyad," "we," "us," or "our") is a care-management company contracted with Medicare Advantage, D-SNP, and Medicaid health plans to deliver dementia and caregiver support services. This policy describes how we handle information we collect through dyad-health.com and its sub-pages (the "Site") and in the course of waitlist and general business communications.
What this policy is not. This policy does not describe how we handle Protected Health Information ("PHI") we receive under a Business Associate Agreement from a health plan or provider. Our handling of PHI in the course of delivering care is governed by the HIPAA Privacy & Security Rules, our HIPAA Notice of Privacy Practices, and the specific Business Associate Agreement we sign with each plan partner. § 02Information we collect on the Site
Information you give us
- Waitlist form. Your name, email address, the ZIP code of the person you care for, and (optionally) the insurance plan of the person you care for.
- Contact forms and emails. Anything you send to hello@dyad-health.com.
Information collected automatically
- Standard web logs. IP address, user agent, referring URL, pages visited, time on page. Retained for up to 90 days for security and analytics, then aggregated or deleted.
- Strictly necessary cookies only. We do not use advertising cookies. We do not use third-party behavioral-tracking pixels on pages intended for caregivers. Any analytics we add is privacy-preserving and, on caregiver-facing pages, IP-anonymized.
§ 03How we use your information
We use information you provide on the Site to:
- Follow up with you when a plan partner is live in your area (waitlist).
- Respond to inquiries from health plans, press, and candidates.
- Improve the Site and understand which content is useful to visitors.
- Meet our legal obligations.
We do not sell your personal information. We do not share it for marketing purposes. We do not use your information to train third-party AI models.
§ 04How we share your information
We share limited Site-collected information only with:
- Service providers we use to run the Site (hosting, email delivery, form handling). Each is bound by contract to use the information only as directed by us.
- Our care team internally, if you submit a waitlist form and a plan partner becomes live in your area — so we can reach out.
- Law enforcement or regulators when required by law, subpoena, or valid legal process, after appropriate review.
- A successor entity in the event of a merger, acquisition, or corporate reorganization, provided the successor agrees to protect your information consistent with this policy.
§ 05Your rights
Depending on where you live, you may have the right to:
- Know what personal information we have collected about you.
- Request a copy of that information.
- Correct inaccurate information.
- Delete your information, subject to legal retention requirements.
- Opt out of any "sale" or "sharing" of your personal information — though Dyad does not sell or share personal information for cross-context behavioral advertising.
- Not receive discriminatory treatment for exercising any of these rights.
To exercise any of these rights, email hello@dyad-health.com. We will verify your identity and respond within the timeframe required by applicable law.
§ 06Data retention
We keep Site-collected information only as long as we need it for the purpose for which it was collected, or as long as required by law. Waitlist submissions are retained until you ask us to delete them or until two years have passed without the corresponding plan partner becoming live in your area — whichever is sooner.
§ 07Security
We use commercially reasonable administrative, technical, and physical safeguards to protect the information we hold. No system is perfectly secure, and we cannot guarantee that a determined attacker will be unsuccessful. If we learn of a breach that materially affects your information, we will notify you consistent with applicable law.
§ 08Children
The Site is not directed to children under 13. We do not knowingly collect personal information from children under 13. If you believe we have received such information, please email hello@dyad-health.com and we will delete it.
§ 09Changes
We may update this policy from time to time. If we make a material change, we will post a notice on the Site and update the effective date at the top of this page. We will not retroactively apply a less protective version of the policy to information we have already collected.
